Becoming a Daughter Again
Becoming a Daughter Again
Becoming a Daughter Again
Clara loved caring for her mom, Teresa. What broke her down wasn’t love—it was logistics. Neurology, primary care, therapy schedules, refills, portals, transportation, forms. By the time she sat down at night, she was still piecing together instructions from different offices.

The challenge
Nothing was in one place. Results weren’t in the chart before appointments. Referrals and approvals dragged. Prior authorizations bounced between inboxes. The medication list never looked the same in each portal. PT, OT, and speech were helpful, but the plans didn’t always connect. Clara wanted to keep her mom safe and steady—and still have a life of her own.

The Journey
The Journey
The Journey
Clara didn’t need to be “more organized.” She needed a partner. A private medical liaison—ECFMG-certified, with clinical background—coordinated quietly in collaboration with her physicians, made sure her questions were heard, and kept the plan moving. The goal was simple: fewer scrambles, more time together.
Starting Point
With consent, we:
• Collected releases and unified records across providers
• Built one accurate medication list and synced the preferred pharmacy for on-time refills/delivery
• Pre-scheduled labs and imaging ordered by her clinicians before upcoming visits
• Created a shared calendar that fit Clara’s work and family schedule, with travel buffers and reminders
• Opened a secure client portal with a family view: records, after-visit summaries, physician instructions, approvals, timelines—everything in one place
• Assembled a one-page “Care at a Glance” (contacts, meds, allergies, routines) for home aides and emergencies
Our Approach
We handled outreach to neurology, primary care, and therapy teams (PT/OT/speech), tracked referrals and approvals, monitored prior-auth status, and ran each visit end to end: pre-briefs before (context, current med list, key questions) and after-visit reconciliations (records updated, instructions captured, labs/imaging/therapy bookings)—so nothing was missed.
Alongside coordination, we layered evidence-based wellness that fit Teresa’s day and her clinicians’ guidance: simple meal structure and hydration cues, gentle movement and daylight time, a calming evening routine, and caregiver-friendly stress-down practices for Clara. Support was available seven days a week.

“I stopped being the coordinator and got to be her daughter again. One plan, one place, and people who made sure nothing was missed.”

Progress
Within weeks, the open loops shrank. Results were in charts before visits. Therapy calendars lined up. Refills arrived on time. After-visit steps were already booked. Clara stopped juggling five portals and started checking one. Evenings felt less like admin and more like time with her mom. The system didn’t change—but how they moved through it did.
