Marcus and Aaliyah ran their days like a relay—work, errands, family, repeat. When Marcus finally made time for a checkup, the labs were a wake-up call: diabetes off track and kidneys under strain. They didn’t want their evenings to become admin—and they didn’t want fear to run the house.

Diabetes care means moving parts: endocrinology, nephrology, primary care, labs, eye and foot exams, refills, devices, and food choices that actually work at home. Results weren’t always in the chart before visits. Prior auths dragged. The medication list looked different in every portal. They spent more time coordinating than being together, and the stress showed.

The Journey
What they needed wasn’t another app; it was calm coordination. A private medical liaison—ECFMG-certified, with clinical background—worked quietly in collaboration with their physicians, making sure Marcus’s questions were heard, priorities came first, and the plan stayed connected.
Starting Point
With their consent, we:
• Collected releases and unified records across PCP, endocrinology, and nephrology
• Built one accurate medication & supplement list
• Synced the pharmacy for on-time refills and delivery
• Pre-scheduled labs ordered by his clinicians (HA1C, kidney panels, urine albumin/creatinine, etc.) ahead of visits
• Booked diabetes eye and foot exams per recommendations
• Opened a secure Couple View portal so everything lived in one place—results, after-visit summaries, physician instructions, approvals, timelines—while preserving individual privacy
• Set a shared calendar around court dates and commutes, plus a protected “no-admin” evening each week
Our Approach
We handled outreach to every office, tracked referrals and approvals, monitored prior-authorization status (including devices/monitoring if ordered), and ran each visit end-to-end: pre-briefs before (context, current med list, top questions) and after-visit reconciliations (records updated, instructions captured, labs/imaging/referrals booked)—so nothing was missed.
Alongside coordination, we layered evidence-based basics—aligned with his care team—that fit their life: consistent mealtimes with attention to carb quality and portions, a short walk together after dinner, a steady wind-down, morning light when possible, and simple stress-down cues. Food stayed familiar and practical; the goal was small changes that stick.

“We stopped arguing with the calendar. One plan, one place, and someone to run it—so we could walk, eat, and breathe like a team.”

Progress
Within weeks, the scramble eased. Results were in charts before appointments. Refills landed on time. Next steps were already booked. The small routines held because they were small—and they did them together. Under his clinicians’ guidance, Marcus’s numbers began to move in the right direction. Evenings felt more like evenings again.
